Product Introduction

In the landscape of osteoporosis treatment, the advent of teriparatide is undoubtedly a revolutionary breakthrough. It breaks the long-standing deadlock in osteoporosis treatment, which "can only slow down bone loss, not actively build bone." As the world's first and only approved bone-forming drug, it acts more like a precise "bone architect." Essentially, it is the active fragment of the first 34 amino acids at the N-terminus of synthetically produced human parathyroid hormone (PTH). By mimicking the physiological rhythms of natural hormones in the human body, it directly awakens the growth potential of bone.

Core Functions:

In vitro and animal models show that teriparatide primarily regulates bone remodeling and systemic calcium and phosphate homeostasis. Under intermittent administration, it can promote the differentiation of osteoblast progenitor cells into mature osteoblasts, prolong osteoblast lifespan, increase the rate of bone matrix synthesis, improve trabecular bone connectivity and cortical bone thickness, and repair damaged bone microstructure.

It acts on the renal tubules, regulating calcium reabsorption and phosphate excretion, indirectly enhancing calcium absorption efficiency through intestinal stimulation, and maintaining a dynamic balance of circulating calcium levels. In scientific research, Teriparatide is frequently used in models of postmenopausal bone loss, glucocorticoid-induced bone injury, fracture healing, and bone defect repair. It is also used to compare the biological differences between anabolic and anti-resorption intervention pathways and to explore the signaling interaction logic between osteocytes and osteoblasts. It is important to note that under continuous exposure, this peptide may shift towards promoting osteoclast activation and accelerating bone resorption; this is a variable that must be controlled in experimental design.

Mechanism of Action:

As a PTH1R receptor agonist, Teriparatide targets and binds to the G protein-coupled PTH1R receptor on the surface of osteoblasts, osteocytes, and renal tubular epithelium, initiating the cAMP-PKA downstream signaling cascade. The biological course of action is entirely determined by whether the signal is intermittent or continuous.

In intermittent pulsed mode, transient activation of PTH1R promotes the directed differentiation of mesenchymal precursor cells into osteoblastic lineages, inhibits osteoblast apoptosis, and preserves a functional osteoblast population. Simultaneously, it downregulates sclerostin, a sclerosing protein secreted by osteocytes, relieving inhibition of the Wnt pathway and amplifying bone formation signals. At the same time, it moderately upregulates RANKL expression. Although osteoclasts are also activated, the rate of bone formation significantly exceeds that of bone resorption, entering the so-called anabolic window and achieving net bone mass gain.

If the molecules persist within the system, the receptor remains in a prolonged activated state, RANKL signaling remains dominant, osteoclasts are overactivated, and bone resorption surpasses osteoogenesis, resulting in bone loss manifestations similar to hyperparathyroidism.

Beyond the skeletal target, receptor activation is transduced to the kidneys, altering the activity of renal tubular transport proteins, retaining blood calcium, and excreting phosphate, thus completing the regulation of systemic mineral homeostasis. The core of this entire mechanism is not simply "promoting bone growth," but rather rewriting the dynamic balance between osteoblasts and osteoclasts within the bone remodeling unit. The timing and concentration of drug administration are the key switches determining the final outcome.

Storage Conditions

Unopened pen: Refrigerate at 2–8℃, protect from light, keep in original box. Do not freeze or place against the refrigerator wall. After opening (from the first injection date): Store upright at 2–8℃. Return to the refrigerator immediately after needle removal. Do not leave the needle in the pen. Discard the entire pen after 28 days, even if only one or two bars of medication remain. The solution must be colorless, clear, and free of particles. If frozen, cloudy, colored, or flocculent, discard immediately. When carrying the medication, use an insulated bag and ice pack. Do not leave the pen at room temperature (>25℃) overnight.

Precautions

Teriparatide also has strict guidelines for clinical application and patient management. The most critical limitation is that its cumulative lifetime use must not exceed 24 months. This regulation stems from early animal studies that showed high-dose long-term administration may increase the risk of osteosarcoma. Although this association has not yet been confirmed in humans, for extreme safety reasons, the medication must be discontinued after 24 months, and a seamless transition to anti-bone resorption drugs such as bisphosphonates should be initiated to "lock in" the hard-won bone mass. In addition, some patients may experience transient orthostatic hypotension after the first injection, manifesting as dizziness or palpitations. Therefore, it is recommended to remain in a sitting or lying position during the injection. Patients with a history of kidney stones, hypercalcemia, or malignant bone tumors are considered absolute contraindications.
