Adamax (N-acetyl-Semax-adamantane, sequence Ac-Met-Glu-His-Phe-Pro-Gly-Pro-adamantane) is the most extreme and unconventional variant in the Semax family. Extracted from the ACTH(4-10) fragment, it retains the core Semax backbone but adds an adamantane cage-like hydrophobic group at the C-terminus and an acetylated cap at the N-terminus. In short, it coats a small peptide, which is easily cleaved by peptidases, with adamantane as a "bulletproof vest," allowing it to survive longer in the bloodstream and cross the blood-brain barrier more effectively. Currently, it has no market approval from any major regulatory agency worldwide, and the label always reads "Research Use Only."
Adamax's most striking design is that it essentially "welds" an adamantane group onto the Semax structure. This rigid, lipophilic cage-like structure acts like a "bulletproof vest" for the peptide, not only maximizing its resistance to enzymatic degradation but also significantly enhancing its blood-brain barrier penetration. In research models, it not only strongly upregulates the sensitivity of BDNF (brain-derived neurotrophic factor) and TrkB receptors but also regulates dopamine and norepinephrine. The cognitive enhancement and physical stamina boost shown in animal experiments, far exceeding that of ordinary Semax, is truly exciting for researchers conducting neuroprotective studies.
Most influenza A strains are naturally resistant to current circulating strains, and the CDC has long since stopped recommending them as first-line treatment. Central nervous system side effects are troublesome: insomnia, dizziness, nightmares, anxiety, irritability; in the elderly, hallucinations and delirium often occur; nearly one in ten people who use livedo reticularis long-term will develop lesions; and sudden edema, shortness of breath, and rapid weight gain are signals of latent heart failure. 95% is excreted unchanged in the urine. If the dosage is not reduced due to kidney problems, it can accumulate and cause toxicity. Blood drug concentrations exceeding 1.5–2.0 μg/mL can cause delirium and seizures. Abruptly stopping PD treatment will cause a rebound worsening; a gradual taper is necessary. Contraindicated in patients with epilepsy, breastfeeding mothers, and children under 1 year old.
Adamax is extremely sensitive to temperature. In lyophilized powder form, it must be kept in a freezer at -20°C. Once reconstituted with antibacterial water, it must be immediately transferred to a refrigerator at 2–8°C and can only be stored for a maximum of 1 to 2 weeks. Repeated freeze-thaw cycles must be avoided, as the peptide structure cannot withstand such stress.
Precautions
PD: Starting dose 100mg once or twice daily, maximum dose 400mg/day; Antiviral: 200mg/day divided into 1–2 doses. For influenza prevention in patients over 65 years of age, halve the dose.
The last dose should be taken before 4 PM, otherwise, you will be awake all night.
Renal insufficiency (decreased CrCl) requires dosage reduction based on clearance rate; it's not about "taking one less tablet," but potentially one tablet every other day.
Combined use with anticholinergics, tricyclic antihistamines, phenothiazines, or antihistamines can amplify dry mouth, urinary retention, and hallucinations; combined use with central nervous system stimulants (including large doses of caffeine) may cause seizures; alcohol exposure doubles dizziness; alkaline urine (sodium bicarbonate, large amounts of dairy products) can lead to poisoning.
Do not drive or climb ledges while taking this medication; impaired concentration is a serious matter.
If hallucinations, purpuric spots, ankle edema, shortness of breath, depression, or suicidal thoughts occur, stop medication and consult a doctor immediately; do not wait for "adaptation."
This medication is contraindicated for pregnant women (Category C), breastfeeding mothers, and children under 1 year old; those with a history of epilepsy, congestive heart failure, glaucoma, or severe liver disease should undergo careful evaluation.
