HGH 191aa, or somatropin, is a recombinant human growth hormone. It is a 191-amino acid single chain with two intramolecular disulfide bonds and a molecular weight of approximately 22.1 kDa. Its sequence is completely identical to that of natural GH secreted by the anterior pituitary gland, with no additional methionine residue at the N-terminus, resulting in low immunogenicity.
After entering the body, it binds to the GHR on the surface of target cells, triggering the JAK2/STAT5 cascade, upregulating IGF-1 expression, and driving linear growth and cell proliferation in bone, muscle, and visceral tissues. It also participates in the regulation of glucose and lipid metabolism, increasing free fatty acid concentration and exhibiting a tendency to antagonize insulin sensitivity.
Clinically, it is used for confirmed growth hormone deficiency (children and adults), Turner syndrome, and dwarfism associated with chronic renal insufficiency. Off-label uses include severe burns and short bowel syndrome, requiring strict medical supervision. It is ineffective orally as proteolytic enzymes break it down; therefore, it must be administered subcutaneously, usually to the abdomen or lateral thigh, once daily or as directed by the treatment plan.
