As a dual agonist of glucagon-like peptide-1 (GLP-1) and glucagon receptors, Survodutide has demonstrated significant efficacy in clinical studies of obesity and metabolic-associated steatohepatitis (MASH). However, any biologically active drug comes with specific contraindications. Based on current clinical trial inclusion and exclusion criteria and the drug's mechanism of action, the following clinical scenarios are explicitly listed as contraindications for Survodutide use or require extreme caution.
I. Personal or Family History of Medullary Thyroid Carcinoma
Reason for Contraindication: Survodutide carries a black box warning for GLP-1 receptor agonists. Patients with a personal history of medullary thyroid carcinoma (MTC), a first-degree family history, or multiple endocrine neoplasia type 2 (MEN-2) are strictly excluded from all relevant clinical trials.
This contraindication stems from early preclinical toxicology findings: GLP-1 receptor agonists can induce thyroid C-cell tumors in rodents. Although a clear causal relationship has not been established in humans, this contraindication applies to the entire drug class, including Survodutide, for safety reasons.
II. Severe Hepatic Impairment and Decompensated Liver Disease
Reason for Contraindication: Although Survodutide has shown potential in treating MASH, its clinical trials have explicitly excluded patients with specific stages of liver disease. Specifically:
Severe Hepatic Impairment (Child-Pugh C): These patients were excluded from the primary trial, partly because the glucagon receptor agonist activity theoretically carries uncertain risks in severely decompensated livers.
Decompensated Liver Cirrhosis Events: Patients with a history of ascites, hepatic encephalopathy (≥ grade 1), or portal hypertension-related upper gastrointestinal bleeding are not eligible for enrollment.
Specific laboratory abnormalities include: aspartate aminotransferase (AST) and/or alanine aminotransferase (ALT) ≥ 5 times the upper limit of normal (ULN), albumin < 3.5 g/dL, international normalized ratio (INR) > 1.3, total bilirubin ≥ 1.5 times the ULN, and platelets < 100,000/μL.
Mechanism analysis: Severe hepatic synthetic dysfunction implies decreased drug metabolism and protein binding capacity, potentially increasing drug exposure and the risk of adverse reactions. Simultaneously, glucagon receptor agonism may affect hepatic hemodynamics, requiring high vigilance in patients with esophageal varices or portal hypertension.
III. Severe Renal Impairment Reason for contraindication: Although studies are exploring the effect of Survodutide on improving proteinuria in patients with chronic kidney disease (CKD) (ARTIST-CKD trial), most current phase II clinical trials exclude patients with severe renal impairment whose estimated glomerular filtration rate (eGFR) is less than 30 mL/min/1.73 m².
Reasons: For patients with end-stage renal disease or severe renal insufficiency, the pharmacokinetic data for this drug are insufficient, and there is a lack of clear basis for dose adjustment; therefore, it is listed as a relative or absolute contraindication.
IV. Type 1 Diabetes
Reason for Contraindication: A diagnosis of type 1 diabetes mellitus (T1DM) is one of the exclusion criteria for Survodutide-related trials.
Scientific Logic: Survodutide contains glucagon receptor agonist activity, and glucagon plays a key role in regulating endogenous glucose production. In T1DM patients lacking endogenous insulin secretion, the use of this drug may interfere with the already fragile glycemic balance, increasing the risk of glycemic fluctuations or ketoacidosis. Currently, there is a lack of safety and efficacy data in this population.
V. History of Active or Previous Pancreatitis
Reason for Contraindication: A history of acute pancreatitis, chronic pancreatitis, or idiopathic acute pancreatitis within 180 days prior to screening is an exclusion criterion.
Background Risks: Post-marketing surveillance of GLP-1 receptor agonists has reported cases of acute pancreatitis. Although large-scale randomized controlled trials have not shown a clear increase in incidence, such patients are cautiously excluded from trials for safety reasons.
VI. Uncontrolled Cardiovascular Disease and Pregnancy Status
Recent Cardiovascular Events: Patients who experienced myocardial infarction, unstable angina, or stroke within the 3 months prior to enrollment were excluded. Severe heart failure (NYHA class IV) was also included in the exclusion criteria of some trials.
Pregnancy and Lactation: Based on animal reproductive toxicity studies showing that GLP-1 agonists can cause fetal harm at clinically relevant exposure levels, and the lack of human safety data, pregnant and lactating women are explicitly listed as contraindicated populations.
VII. High Risk of Gastrointestinal Adverse Events and History of Intolerance
Reason for Contraindication: Gastrointestinal adverse reactions are the most common adverse events associated with Survodutide. In Phase III trials, the incidence of gastrointestinal events (nausea, vomiting, diarrhea) in the Survodutide group was as high as 80.9%–89.7%, with approximately one-fifth (17.8%–20.2%) of subjects discontinuing the drug as a result.
Population limitations: For patients with severe gastroparesis, active inflammatory bowel disease (IBD), or a recent (within 6 months) history of gastrointestinal ulcers or bleeding, use of this drug may lead to a rapid deterioration of their condition due to the combined effects of delayed gastric emptying and pro-inflammatory risks.
Conclusion: In summary, the main clinical contraindications or precautions for Survodutide revolve around the risk of thyroid C-cell neoplasia (MTC/MEN-2), decompensated liver disease (cirrhosis with ascites/hepatic encephalopathy), severe renal insufficiency (eGFR <30), type 1 diabetes, active pancreatic disease, uncontrolled severe cardiovascular disease, and pregnancy. The scientific basis for these contraindications stems from the pharmacological effects determined by the drug's dual GLP-1/Glucagon agonist mechanism of action, as well as safety data accumulated in preclinical and clinical trials. Prior to the formal approval of Survodutide for marketing, its indication population will strictly adhere to the inclusion and exclusion criteria defined in Phase III registration studies (such as the SYNCHRONIZE and LIVERAGE series trials).
