MT2 (Melanotan II) is a synthetic cyclic peptide compound. As an analogue of alpha-melanocyte-stimulating hormone (α-MSH), it exerts various physiological effects by activating the melanocortin receptor family. Since its discovery, MT2 has attracted widespread research interest in areas such as the regulation of pigmentation and the improvement of sexual function. This article systematically outlines the mechanism of action and potential benefits of MT2 from a scientific perspective.

I. Molecular Mechanism of MT2
MT2 is a non-selective melanocortin receptor agonist, exhibiting agonist activity across four receptors: MC1R, MC3R, MC4R, and MC5R. This non-selective nature allows MT2 to influence multiple physiological systems simultaneously.

From the perspective of signaling pathways, MT2 activates G protein-coupled receptors, promoting the activation of adenylyl cyclase (AC). This leads to increased intracellular cAMP levels and the activation of protein kinase A (PKA), ultimately regulating downstream gene expression through the phosphorylation of transcription factors such as CREB. This classic "melanocortin receptor → Gs → AC → cAMP → PKA → transcriptional regulation" cascade serves as the molecular basis for the diverse biological effects of MT2.

II. Regulation of Pigmentation
MT2 was originally developed to promote skin pigmentation. Its mechanism involves activating MC1R and-via the aforementioned signaling pathway-upregulating the melanogenesis regulatory network centered on MITF. This induces the expression of tyrosinase and related enzymes, thereby promoting melanin synthesis.

In clinical research, MT2 has been investigated as an adjunct to narrowband UVB (NB-UVB) phototherapy for repigmentation treatment in patients with stable non-segmental vitiligo. A randomized, double-blind, placebo-controlled Phase 2 clinical trial is currently evaluating the efficacy of MT2 combined with NB-UVB phototherapy for adults with stable non-segmental vitiligo.

III. Improvement of Sexual Function
The ability of MT2 to enhance sexual function is one of its most notable findings. This effect was initially discovered by chance during research into its pigmentation-inducing properties: male subjects receiving MT2 injections reported unexpected erectile activity.

Studies indicate that MT2 acts centrally via the hypothalamic MC4R signaling pathway-a mechanism distinct from that of PDE5 inhibitors (such as sildenafil), which operate through the peripheral nitric oxide/cGMP pathway. A double-blind, placebo-controlled crossover study involving 10 patients with psychogenic erectile dysfunction found that 8 patients in the MT2 treatment group achieved clinically detectable erections; the mean duration of penile tip rigidity exceeding 80% was 38 minutes, compared to just 3 minutes in the placebo group.

Notably, this discovery regarding MT2 directly led to the development of bremelanotide (PT-141)-an N-acetylated cyclic analog approved by the US FDA for the treatment of female sexual dysfunction in premenopausal women.

IV. Potential Benefits for Metabolic Regulation
As an MC4R agonist, MT2 also shows promise in the regulation of energy balance. MC4R is primarily expressed in the central nervous system and plays a role in regulating appetite and energy expenditure. Animal studies have shown that central administration of MT2 suppresses feeding behavior in mice. This finding provides a valuable pharmacological tool for the development of weight-loss drugs targeting MC4R.

V. Safety and Regulatory Status
Despite the aforementioned potential benefits, safety concerns regarding MT2 cannot be overlooked. MT2 has never been approved for human use by any national drug regulatory agency. Published case reports have documented various adverse reactions following MT2 use, including:

Skin-related: formation of new moles, rapid darkening and enlargement of existing moles, eruption of atypical moles, and the development of melanoma

Other systems: nausea, facial flushing, spontaneous erections, muscle damage, etc.

The Skin Cancer Foundation has issued a specific warning emphasizing that MT2 is not FDA-approved for any use and its safety has not been established. The Drug Office in Hong Kong has also issued a safety alert, noting that there are no registered pharmaceutical products containing MT2 in Hong Kong.

Conclusion
As a pharmacological tool, MT2 holds significant value for understanding the physiological functions of the melanocortin system. Research into the mechanisms by which it regulates pigmentation and improves sexual function has directly driven the development of more selective and safer drugs. However, commercially available MT2 products lack quality oversight, with no guarantees regarding purity, dosage, or composition; consumers should be fully aware of the potential risks and make informed decisions under professional medical guidance.
